Accredited Application Form
Before you commence this Application Form, please ensure you have the following documents and ID ready to upload:
- Colour copy of both sides of your Photo Identification
- You need one form of government issued photo identification that shows at least your name and photograph.
- For example, Drivers License, Australian State or Territory Proof of Age/Photo Card, National ID Card, Passport (without passport number).
- Unique Student Identifier (USI) Number (for Australian residents and citizens only).
- Create a new USI or retrieve your exisiting USI from https://www.usi.gov.au
- Certified digital copies of previous Australian qualifications (if any).
- Through the student portal in the USI Registry System, you can give permission to a Registered Training Organisation (RTO) to view your VET transcript to arrange for the recognition of prior learning/credits transfers, and verify your training records. Our RTO details to use in the USI access are: Provet Pty Ltd. RTO: 31424
- Alternatively, provide a certified digital scan of the transcript.
- Certified Digital copies of your mentor’s qualifications
- You mentor for this course must be your supervisor or another senior person you work closely with.
- The mentor must hold the same qualification you are studying, an equivalent qualification, or a higher qualification.

NB: Document copies may be certified by a registered veterinarian (who is not named in the document). The certified copy must include the statement “I certify that this is a true copy of the original document“. The certifier must also include their full name, signature, date, registration number and qualification or occupation which makes them eligible to certify documents (e.g., state “BVSc” or “veterinarian”)
